Periurethral constrictorThe Silimed Periurethral constrictor was developed by Dr Fabio Vilar specifically to treat paediatric patients with deficient bladder sphincter function. The device is primed with saline then implanted firmly around but without constriction around the bulbar-urethra or bladder neck. The port is implanted in a space where it can be punctured percutaneously to inject further saline after healing has taken place. After 6-8 weeks, the patient returns for the system to be activated. This requires filling the bladder then injecting further saline into the system through a Huber needle for as long as leaking occurs when the patient coughs. To avoid the risk of complications from ischemia, the pressure should be relieved for about two months each year. The silicone device is comprised of two parts: constrictor boby and injection port with tube. The body is in the shape of an open ring with an adjustable strap to fit around the bulbar urethra or bladder neck. The injection port is designed to accommodate a fine Huber needle. There are three sizes:
